We aim to provide a person centred, tailor made support service that you need to maintain your well being and to continue to live longer at home and in the community and as independently as possible.

Our Rehabilitation, Enablement and Care service is personalised for you to help those:
• planning to leave hospital
• recovering from an illness or
• having difficulties with day to day tasks around the home.

The community independence service works with you to develop a personalised care plan based on your needs and what you want to achieve, helping you to do as much as you can for yourself and setting goals such as:
• regaining your confidence
• rebuilding your skills
• increasing your independence.
• stay healthy and safe
•Assist you to re-engage and be involved in family and community life and encourage you to do as much for yourself to maintain your independence.
• Help and assist you to work out with you how best to achieve what you want If you would like a tailor made person centred care or support service as we believe every individual deserves the right care and support.

We provide a wide range of services and pride ourselves on providing a high standard of service Specialising in adults with mental health, cognitive difficulties and substance abuse, learning difficulties and ABI and TBI brain injury rehabilitation.

Our existing team has a history of working closely and working towards rehabilitation goals with service users from forensic and clinical settings. Skilled in crisis intervention, challenging behaviour management, team leading and case management. Strong support professionals with a passion for health and social care, whom remain focused on delivering a high standard of person centred care to service users. As a provider we continually demonstrate a positive attitude towards the service, including clients, families, staff and others.
