What services do we provide?
We provide professional, high-quality care and support for you in your home. Whether you require occasional visits for social activities, or daily assistance with practical tasks to help build your future, our goal is to help you live a dignified, independent and full life in your own home, and out in the community.
Our support packages are aimed at helping you understand your condition including your strengths and weaknesses and helping you to live an independent life. This also includes help with finding useful and effective strategies for day-to-day problems as well as any related conditions such as; anxiety, obsessive compulsive tendencies and challenging behaviours.
As our packages are tailored to each individual's needs each care plan is unique, meaning services can range from daily tasks through to more intensive packages for client’s with long term conditions. As such this is not an exclusive list of services;
Daily tasks to promote independent living:
1. Helping your find and implement strategies and coping mechanisms for areas or situations that you struggle with.
2. Assistance with preparing and cooking meals, including help with balancing your diet
3. General household cleaning
4. Shopping
5. Banking, paying bills and budgeting
6. Escorting to appointments
Helping you build your future:
1. Guidance in helping you explore your strengths and weaknesses and creating a plan of actions and desired outcomes to help you meet your goals.
2. Assistance with pursuing a career including help finding voluntary or paid work
3. Accompanying you to college, volunteering or work
4. Assistance with homework
5. After school sitting service
Social activites:
1. Providing support and encouragement in creating and building on friendships
2. Accompanying you on social activities and day trips
3. Helping you to play a part in your community by finding local groups or clubs you can join
4. Help with pursuing hobbies and leisure or sporting activities including encouraging a healthy lifestyle
All personal care (as required) including:
1. Dressing and undressing
2. Prompting and assisting with medication
3. Assisting with toileting needs
4. Bathing/showering/ shaving and oral care
What happens when you call us?
Whether you have been referred to us by social services or contacted us directly, information about you will be handled sensitively and in confidence. A member of our staff will discuss our services with you and gain an understanding of the type of services you require, and ensure that our services can meet your needs. Following this we will arrange a time which best suits you for a member of specially trained staff to visit you in your home. On this visit we will answer any of your questions, explain all of your options for services and fill out a Needs Assessment. This will give you a better understanding of what we offer and how we deliver it, and allow us to create a summary focused specifically on your individual needs. Having assessed your needs we will prepare a service user care plan which will include our recommendations for your care and how you can expect us to deliver it. This will include details such as the times of visits, special tasks to be performed and the general objectives of the care provision. Over time, your needs may change. You may need more or less care, the type or pattern of service may have to be varied and new risks may become apparent. You can be confident that we will keep your needs under review and make decisions about your care together.
All our services are available through self-funding, direct payments and personal budgets.
