The team consists of 28 experienced Social Care workers with a wide variety of skills.
Apuldram Supported Living Aims and Objectives
1. To support individuals to live independently in the community
The Apuldram Centre will support individuals to live independently in the community.
- Support to maintain tenancies
- Support to make and attend medical appointments
- Support to carry out the tasks of daily living
- Support with understanding rights and responsibilities
2. To deliver services in line with The Reach Standards in Supported Living
The Reach Standards:
- choose who I live with
- choose where I live
- have my own home
- choose how I am supported
- choose who supports me
- get good support
- choose my friendships and relationships
- choose how to be healthy and safe
- choose how to take part in my community
- get help to make changes in my life
- have the same rights and responsibilities as other citizens
3. To deliver person centred packages of support, tailored to the needs of each individual
The Team will work closely with each individual to develop Support Plans and Risk Assessments that meet their needs in a proactive and holistic way. We will work closely with Care Managers, Occupational Therapists, Advocates and Family members to ensure that the needs of the individual are at the centre of what we do.
4. To support and assist people to maintain their dignity
The Supported Living Team will offer prompting and supervision for individuals that need assistance with the tasks of daily living.
Physical assistance will also be available for individuals that need support with
- Eating or drinking
- Toileting
- Washing or bathing
- Dressing
- Oral care
- The care of skin, hair and nails
5. To promote the principles of Rights, Independence, Choice and Inclusion
The Supported Living Team works closely with the Western Community Learning Disability Team.
